- The GENEROUS Model (GENErating cost Reductions fOr U.S. Medicaid) is a voluntary CMS demonstration program running from January 2026 through 2030. It introduces Most Favored Nation (MFN) pricing to Medicaid by using PPP-adjusted net prices from 8 reference countries to calculate supplemental rebates. The benchmark is set by the 2nd lowest price, and manufacturers must achieve a Guaranteed Net Unit Price (GNUP) calculated as: WAC minus (GNUP plus URA).
